Which forms should
I use?

Please
click below for the appropriate forms to complete electronically and
submit to the Biological Safety Officer via email. Print signature
pages from below (Part 5 of the original application and (if applicable)
the last page of the Bloodborne Pathogens Attachment, BBP), sign and
submit the hardcopy via campus mail to the Biological Safety Officer
(Marcia Finucane, BSO; 252 E. Maxwell St., Speed Sort 0314). Please
contact the Biosafety Officer, Marcia Finucane, at 257-1049 or mfinu2@email.uky.edu
for assistance.
Please
note that Parts 1-5 need only be completed once per
application and there is a difference between the animal and plant
Parts 1-5. Please submit separate attachments for each organism or
infectious agent being used. (Contact the Biological Safety Officer
for special instructions if there are numerous organisms or agents
being used.)
To complete the
forms, on the View menu, point to Toolbars, and then click Forms to
open the Forms toolbar. Click the padlock icon that appears on the
Forms toolbar. This allows you to fill in the fields on the registration
forms. Once you have completed filling in the registration forms,
on the File menu click Save As. In the File name box, enter a new
name for the file. Click the Save as type list, and then click the
Rich Text Format file type. Click Save.

protocol has been provided as a guide to the completion of the

for any additional questions you may have regarding completion of
the forms.
